a consultation with her. It was very easy to
converse with her and she was able to Health online

Nichola shines bright
Nichola Diamond from the Maidstone health and social care co-ordinators,
has been supporting me for a matter of months as I was struggling at
home. She came and completed an assessment and liaised directly with social services
and I now have carers coming in three times-a-day to support me in the home. Nichola
also put me in touch with the complex care nurse who supports me with my diabetes.
