What we do
The service offers a robust way of supporting people who make frequent use of health services, in particular A&E, primary care and mental health services.
We can help you practically and support your emotional health and wellbeing with lots of different things, such as:
- being at the end of the phone if you need to talk
- guiding you through housing problems
- linking to other people or services, for example a local support or health group
- introducing you to an activity or group you may like to join
- giving advice on health or social worries
- talking through ways to solve problems
- understanding letters, for example benefits letters
- attending health appointments and work meetings with you
- talking to your family, GP and other healthcare professionals.
If you fancy an informal chat, we can help with that too – we are happy to come to your home or meet at a place convenient to you.
Under-18s service
Our Under-18s service supports families with children and young people who frequently attend A&E or the GP. We can help with the following:
- being at the end of the phone if you need to talk
- support with communication, language barriers and medical terminology
- collaborating with other professionals where there are complex health needs
- linking to other services
- introducing you to an activity or group you may like to join
- giving advice on health or social worries
- talking through ways to solve problems
- attending health appointments or any other relevant meetings
- talking to your GP and other healthcare professionals.
