Who we are
We are a multi-disciplinary team that includes occupational therapists, physiotherapists and therapy assistants. We work within patients’ homes and rehabilitation groups to ensure effective discharge planning and a seamless rehabilitation pathway. We provide holistic assessment of patients’ needs and help patients achieve maximum independence in daily living activities and improved well-being.
What we do
We provide short-term rehabilitation to enable patients, where possible, to remain in the community rather than be admitted to hospital or long-term care, and to live as independently as possible. We have close links with Acute Hospitals to ensure patients are able to continue their rehabilitation in the community following discharge from hospital.
The team can assess for and provide suitable equipment and home adaptations to increase independence. We assess and devise individual exercise programmes to improve muscle strength, mobility and balance. Our treatment plans are based on patient-centred goals which are aimed at improving patients’ ability to perform daily activities, such as walking, climbing stairs and self-care. We give advice and information on issues relating to patient safety and self-management.
We actively involve patients and family/carers in all aspects of care and can assess the need for and refer on to other agencies and professions where necessary.
