Who we are

We are team of physiotherapists, occupational therapists, speech and language therapists, dieticians, nurses, rehabilitation assistants and administrators who provide rehabilitation for adults with progressive and non-progressive neurological disability. This may include stroke, traumatic brain injury, multiple sclerosis (MS) Parkinson’s disease and motor neurone disease. The team has close links with the neuro-psychology service and works closely with social services, hospitals and the voluntary sector, including the MS Society and Parkinson’s UK.

What we do

We support discharged patients who may have had a stroke or brain injury. The team also helps support people who have a long-term condition such as MS.

We work in the most appropriate community setting, which can be at home, in a day centre or in an out-patient clinic. The team runs a range of therapy groups including balance and exercise and fatigue management (West Kent only). The team works with the patient and family to ensure rehabilitation is carried over into everyday activities.

How do you use this service?

You need to be referred by a GP or healthcare professional.
