Supporting care homes is worthy of award

Kirsty Matthews, Medicines Optimisation in Care Homes Pharmacist, is a finalist in the pharmacy and medicines optimisation category for her work to make sure patients have the right medicines to help keep them safe and well.

She started work on theproject after NHS England and Improvement asked community health services and GPs nationally if they could give greater support to care homes, to help them during the COVID-19 pandemic.

The service carried out medication reviews for patients who left hospital and went to a care home. At the time of launching the pilot service Kirsty was on secondment to KCHFT from Kent and Medway Clinical Commissioning Group.
