This community clinic supports people living with type 2 diabetes. It is a multidisciplinary approach to support individuals to live well with their diabetes.
Our clinic includes a diabetes practitioner and a diabetes specialist dietitian. There will also be someone from your GP practice, usually a practice nurse or clinical pharmacist.
What is type 2 diabetes?
Type 2 diabetes is a common condition which causes the level of sugar (glucose) in the blood to become too high. This is because the insulin your body is producing isn’t enough or it can’t work properly (known as insulin resistance).
High blood glucose levels, if untreated, can result in unpleasant symptoms such as excessive thirst, needing to pass a lot of urine and extreme tiredness.
What can I expect?
You will have an initial appointment with a diabetes practitioner to discuss blood glucose levels, current medications, current practices, address any concerns and to determine if any changes can be made. This appointment will last for up to 45 minutes.
You will then have an appointment with the diabetes specialist dietitian to discuss diet, lifestyle and overall wellbeing. We can also signpost to other services such as social prescribing or talking therapies, if appropriate. This appointment will be up to 45 minutes.
Your total appointment time will be one hour, 30 minutes.

We will help support you to manage your diabetes to the best of our ability along side your current lifestyle and personal goals.
The purpose of all diabetes treatment is to reduce the risk of the long-term complications of diabetes, such as heart attack, stroke, amputation, blindness, kidney failure and nerve damage.
Although this is a long clinic, we hope it will give you the opportunity to discuss any concerns around your diabetes management.
