Referrer please note we do not accept patients assessed for malnutrition with a MUST score of 0 or 1, Please see professional resources below for supporting information to address malnutrition risk for MUST 0 and 1.

Professional resources

Community Dietetics Team are not commissioned to treat the following conditions. Where possible, we have included links or information which may help direct you to the most appropriate services.

  • Those under 18 years of age – refer to Community Dietetics Paediatric Team using the Community dietetics paediatric referral form
  • Oncology patients if currently under active treatment under East Kent Oncology and access to an east Kent oncology dietitian - Oncology Dietetic Team referral form
  • Oncology patients if in west Kent refer to Maidstone and Tunbridge Wells oncology dietetics team contact mtw-tr.nutritionanddietetics@nhs.net
  • Tube fed adults, refer to Clinical Nutrition and Dietetics HEN Team using the CND HEN referral form
  • Malnutrition assessment scoring ‘MUST’ 0 OR 1 (signpost instead to relevant online first line nutrition information resources Managing Malnutrition: Leaflets for Patients and Carers (malnutritionpathway.co.uk))
  • Obesity with BMI<40kg/m2 – refer to OneYou Kent One You Kent - Kent County Council
  • Obesity of any BMI requiring GLP-1 (glucagon-like peptide 1agonists).
  • Pre and post-bariatric surgery – accepted in east Kent only
  • Pre-diabetes - advise self referral to: NHS Diabetes Prevention Programme
  • Type 1 and gestational diabetes – accepted in east Kent only
  • Newly diagnosed Type 2 diabetes or needing type 2 diabetes dietary update – refer to Spirit Healthcare responsible for DERIK group bookings: bookinghub@nhs.net
  • Type 2 diabetes in west Kent excluded if prescribed 3 or more oral hypoglycaemic agents and/or insulin - refer to Maidstone and Tunbridge Wells NHS Trust Diabetes Dietitians: Diabetes Centre mtw-tr.diabetesmgh@nhs.net. (These type 2 diabetes referrals are accepted if in east Kent).
  • General healthy eating, vegetarian, vegan diet etc.
  • Arthritis (unless for weight management meeting criteria of BMI>40kg/m2)
  • NAFLD (unless meeting criteria of BMI>40kg/m2)
  • Epilepsy
  • Eating disorders – refer to NELFT eating disorder service
  • ‘Disordered eating’ which relates to a mental health condition – refer to GP to consider referral to relevant mental health service
  • ARFID – refer to GP to consider referral to relevant mental health service.
  • Low FODMAP diet for Small intestinal bowel overgrowth (SIBO)
  • Kidney stones – refer to EKHUFT Renal Dietetic Service
  • Renal – patients with chronic kidney disease (CKD) stage 4+. Refer to EKHUFT Renal Dietetic Service.
  • Ketogenic diet – refer to London Hospitals tertiary centre
  • Inborn errors of metabolism – refer to London Hospitals tertiary centre

We receive high numbers of referrals into the service. To enable us to process your referral appropriately please complete this form as fully as possible. Referrals with insufficient information will be returned to the sender for revision.
