A new electronic care record can now provide healthcare professionals with a joined-up view of a patient’s care and treatment.
The Kent and Medway Care Record (KMCR) gives clinical staff at KCHFT access to their patients’ records from multiple health providers.
These include four main, acute hospital trusts, three community-based providers, two mental health services and 255 GP practices, in addition to Kent County Council and Medway Council’s children and adult social care teams.
This is important because it helps smoother working and should make sure patients only have to tell their story once.
The next stage of the project is to give Kent and Medway residents access to their own patient records through the KMCR patient portal; this is scheduled to launch next year.
The KMCR – which went live on 1 May – will help our healthcare experts make better decisions for the people in their care.
