Who we are

We sit in the Community Learning Disability Team and cover the whole of Kent and Medway. We support Kent and Medway clients who are currently living in mental health hospitals outside of Kent and Medway to transition back into Kent and Medway. We are a small multi-disciplinary team, consisting of a speech and language therapist,

community learning disability nurse and occupational therapist. We are managed by a community learning disability nurse.

We work closely with local commissioning groups, ICB, local authority and other stakeholders.

What we do

We can support people with a learning disability, who are in a secure or mental health hospital, return to Kent and Medway with the aim of providing a safe and timely discharge.

We offer support to people with a learning disability in Kent and Medway to reduce inappropriate hospital admissions to secure or mental health hospitals.

We provide specialist advice and support to practitioners raising Complex Care Responses (CCRs) and referring to the Dynamic Support Database (DSD).

We provide consultations to colleagues in the Community Learning Disability Service who need support and advice around complex cases.

Our services

Community learning disability nursing

The role of the learning disability nurse in the Transforming Care Team is to support individuals transitioning from hospital to the community. This includes making sure their physical health needs are met through effective care planning and facilitating access to mainstream health services by advocating for reasonable adjustments where necessary. Additionally, learning disability nurses provide support and consultations on positive behaviour support planning and risk assessment development.

Speech and language therapy

The speech and language therapist provides specialist assessment, advice, interventions and resources to support people with communication differences. Speech and language therapy can support people to understand what is happening with their care, make choices and share their thoughts and feelings. The speech and language therapist works closely with colleague and families to share effective communication strategies to support the person.

Occupational therapy

We offer specialist assessment, advice and recommendations to make sure the environment meets each individual’s needs, including specific adaptations. We offer specialist assessments and advice to meet each person’s sensory processing needs where indicated. We provide specialist advice regarding meaningful activities, roles and engaging people with a learning disability in occupation.
