Who we are

We are a multidisciplinary team that includes complex care nurses, advanced clinical practitioners, occupational therapists, physiotherapists, therapy assistants and health and social care coordinators. We provide a range of holistic assessments focused on finding ways to support your health, mobility and independence.

What we do

The Ageing Well Clinic is a client centred service that enables people to age well. It has been designed as a ‘one stop’ clinic made up of our multidisciplinary team of clinicians and co-ordinators, providing you with a confidential one to one consultation that focuses on what matters to you, with the aim of supporting your health, mobility and independence through the ageing process.

Many of the factors that cause people to age differently can be influenced by interventions based on preventative healthcare, lifestyle choices and exercise. Being able to stay healthy in later life is a crucial issue for all of us. No matter what age we are, we are all ageing and getting older. We want to support you to look after your own health, including anyone with frailty.

What is frailty?

In medicine, frailty defines a group of older people who are at risk of adverse outcomes such as falls, disability, admission to hospital or the need for long-term care. People who are frail usually have three or more of five symptoms that often travel together. These include unintentional weight loss (10 or more pounds within the past year), muscle loss and weakness, a feeling of fatigue, slow walking speed and low levels of physical activity.

Frailty also refers to a person’s mental and physical resilience or their ability to bounce back and recover from events like illness and injury.

How do you use this service?

Your GP has referred you to the clinic, as they feel it will meet a number of your needs. You will have access to talk to a variety of healthcare specialists who will be able to assess you and provide information and one to one support in a relaxed and confidential setting

Can I self-refer?

Yes, the Ageing Well Clinic may also be for someone if they are not suffering from any active illness or symptoms currently, but:

  • Feel that they have started to ‘slow up’ i.e. weakness or concerns
    relating to the general ageing processes, which could be physical,
    cognitive or both.
  • Started to experience difficulties or changes to their mobility especially
    outside of the home.
  • Beginning to feel less confident with domestic and physical tasks.
    Noticing that they have started to avoid things due to difficulty managing them?
  • Possibly starting to feel more isolated.

If you are over the age of 65, not housebound and registered with Edenbridge General Medical Practice you can self-refer by contacting the number and email address at the top of the page.

When and where is the clinic held?

We are currently running a clinic for anyone registered with Edenbridge Medical Practice, but hope to expand this to the Sevenoaks PCN area in the future. Please email us on the address given, if you would like to be contacted when this
is available.

The clinic is held twice a month at the Edenbridge Memorial Health Centre. On arrival, please take a seat at the very back of the waiting area and we will come and collect you.

The expected duration of the clinic is approximately an hour and a half to two hours. You are welcome to bring a relative or friend or ask for a chaperone.

What will happen at the clinic?

You will meet different members of our multidisciplinary team including:

  1. A Specialist Frailty Nurse/Advanced Nurse Practitioner who will complete a comprehensive assessment with you to include any concerns that you may have along with a medication review and if needed, a physical examination and memory screening.
  2. A Therapist (Occupational Therapist and/or Physiotherapist) who will undertake assessments to understand what activities you can do (and those you want to do), any current limitations, your goals/motivations and also to offer advice/techniques about how to do something more easily and safely. This could include further review of home environment and advice and assessment of mobility and balance by the OT or PT with an ongoing exercise plan where appropriate.
  3. A Health and Social Care Coordinator who will review any needs you may have and help you to access the services and support you might require and to manage your health and wellbeing, including helping you to navigate care across the health and care system, helping you make the right connections, with the right teams at the right time.
  4. A Social Prescriber, who will link you to a wide range of community groups and services, for social support and to improve wellbeing.

Light refreshments will be provided.

What happens next?

We will send you an appointment letter with a link to the questionnaire below. Please complete this as soon as you receive your appointment letter. If you would like any help with this, then please contact us. We will make a courtesy call to you before your appointment to check if there is anything we can help you with.

What happens after my appointment?

We will send you a summary report and a report to your GP, who will remain responsible for your care. If you require a referral into another service or services following the clinic, this will be arranged for you.

A month after your appointment you will receive a follow up call from a member of the team to see how you are getting on afterwards and to complete a feedback and follow up questionnaire.
