Who are we?
The Community Children’s Nursing Team is made up of specialist paediatric nurses and nursing assistants who are trained to care for children and their families. We work in partnership with parents, carers, specialist hospitals, local hospitals and various other professionals to offer highly skilled nursing care within the child's home and other community settings. We aim to treat children in their homes whenever it is safe to do so and avoid admission to hospital as much as possible.
What do we do?
We offer support to children with complex health needs and their families. These services include:
- planning discharge meetings to make sure safe transition from hospital to home.
- administration of Bolus IV, SC, IM medication
- gastrostomy, naso-gastric support
- wound care
- post-surgery care
- support, information and training
- oxygen care at home
- short term interventions
- nursing intervention for complex long-term health care needs.
How can you access the service?
Our service is open to all children and young people up to the age of 19 in education, with a clinical nursing need. They will need to be registered with a GP in north Kent.
Referrals to this service can be made via a health professional or a family can refer themselves directly.

The team offers 24 hours out of hours visits for end of life care.
